Assessment for newborn child for temp, respiratory rate, sign of dyspnea, abnormal movement, yellow sole Jaundice, local infection, feeding difficulty, disturbed level of conscious, and also for mother if any of these sign present the CHWs will refer the newborn to PHCC for further assessment by medical doctor.
