Medications Procurement
On behalf of Doctors Aid for Medical Activities (DAMA) organization, we are kindly requesting a quotation (RFQ) for the Medications, expressed in USD with delivery to DAMA Office, Villa 709, Italy City 2 Village, Erbil – Iraq.
Please quote your best delivery timeline for the entire content. All supplies must be delivered before [November 21st, 2019] in one shipment otherwise DAMA will not consider split shipment after this date.
All medicines must comply with World Health Organization (WHO) set quality standards, principles of Good Manufacturing Practices (GMP), Good Storage Practices (GSP) and Good Distribution Practices (GDP).
All medication items should have their own Quality control Documents from KMCA or quality control in Baghdad or certificate of origin, otherwise, it will be rejected
Note: (Any bids without required documents will be disqualified).
The expiration date of medication should not be less than October 2020.
DAMA intends to award a firm-fixed-price purchase order or contract as a result of this RFQ with payment terms being within 60 days from delivery and acceptance of the goods ordered with the Invoice at the location specified. If this is not possible, please quote your best payment terms.
The last date to submit the quotation will be on [November 12th, 2019]. Quotations received after the stated deadline may be considered invalid.
You are requested to submit signed and dated offers to the office specified in a closed and stamped envelope at or before the exact time [November 12th, 2019], Quotation can not be submitted via e‐mail.
The supplier must take into consideration the terms and conditions of DAMA organization and approve on the following inside the RFQ form:

Terms of payment (within 60 days from delivery and acceptance of the goods) by cheque or bank transfer.

Delivery place and mode of transport (include freight charges when quoting): DAMA Storage house - Erbil, Italian City 2, Villa 709.

Expected delivery date: Maximum within 7 days, or quote your best delivery timeline.

Packing/labeling and special instructions: Required label tags for the medication packs.
It's recommended to fill the form electronically using DAMA's RFQ excel sheet to avoid calculation mistakes, the form will then printed, signed and stamped by your company.
